抽選券一枚ずつにご記入ください。
１．日本人会会員の方は名前（ローマ字）と会員番号及び電話番号をご記入ください。
２．日本人会非会員の方は、名前（ローマ字）と住所及び電話番号をご記入ください。
＊ 半券は、当選の際に賞品の引き換え券となりますので、大切に保管してください。

If you are a member of the Japanese Association, you may fill in your name and membership number. If you are not a member, please fill in your name, address and telephone number on EACH draw coupon. Please send or drop your draw coupon into the Charity Draw Box placed at the Japanese Association’s Front Office (1F). Please retain the counterfoil for prize collection.
